Referral Guidelines & Scheduling an appointment
Your current primary treating physician, claim adjuster or workers compensation attorney can use our referrals page to submit the referral online. Once the referral is received, the treating office will contact you to gather initial information and schedule your appointment. Following the initial contact, a new patient questionnaire will be sent to you. Please take the time to fill it out completely, this is an essential tool for developing customized care. Scheduling for Physical Therapy / Chiropractic / Acupuncture
Ancillary services must be recommended by a Boomerang Healthcare Provider and are available for current BHC Patients. If you are not a current patient, an appointment with a BHC Provider must be scheduled prior to being scheduled for PT, Chiro or Acupuncture.
Your First Appointment
Please bring the following to your first appointment
- The completed New Patient Packet
- A California driver license or California Identification card
- Insurance card for non Workers Compensation patients
- Current prescriptions (prescription bottles are preferred but the current print out from your pharmacy will also be accepted)
If you are unable to complete the New Patient Packet before your appointment please plan on arriving 30 minutes prior to your appointment time in order to complete the questionnaire in the office. Some physicians will not see new patients without the completed forms which may necessitate rescheduling the appointment.
What to expect during your first appointment
You should plan to spend about an hour and a half during your initial appointment. This includes arriving 15 minutes prior to your scheduled appointment time to complete registration. Your initial evaluation is your first meeting with our physician so please be prepared to provide information regarding previous treatments, physicians, studies and tests. The doctor will discuss treatment options with you and if we are authorized for additional visits beyond the initial consultation, we will attempt to obtain authorization for these treatment recommendations. Your referring physician/insurance carrier/attorney will be sent a copy of your consultation report as well as all future visit notes. Your co-payment is due at the time of registration. We accept all forms of payment.
Rescheduling
Please provide at least 24 hours notice if you need to cancel or reschedule your appointment. Failure to provide sufficient notice may result in fees applied to your account. Treatment visits must be scheduled at least one time every twelve months or a new referral is required.
